To offer our small groups more flexibility when choosing a health plan in both design and cost, we created 22 new HMO, POS, Direct POS and Personal Choice® Flex Series options. Highlights of the new options include:

Hospital Services $400 a day/ max. 5 days ($2,000)
Outpatient Surgery $200
Skilled Nursing Facility $200 a day/ max. 5 days ($1,000)
Emergency Room
(Copay not waived if admitted) 		$125

To maintain competitively priced premiums, only certain options may be paired together. Please note the following combinations:

  • The new C4 options may be paired only with our existing F3, F4, and the new F5.
  • The new F5 option may be paired only with our existing C3 or the new C4 option.
  • For POS and PPO plans, the new F5 option may be paired only with our existing O2 option.
